The Community Health Navigator Opportunity, Maternal Health

The Community Health Navigator (CHN), Maternal Health will be part of a team working together using a community-based approach to address a variety of race-based health disparities with a primary focus on perinatal and infant health outcomes. The community Sabathani serves faces heightened health disparities due to systemic racism and exacerbated stressors such as underemployment and community safety concerns. Stress is a significant factor contributing to harmful behaviors during pregnancy. By providing a holistic approach to stress management tailored to underserved Black pregnant women and new mothers in South Minneapolis, you will be heling pregnant and new mothers better manage stress, improve their social and emotional well-being, and most importantly deliver healthy babies.

Wages and Benefits

$24.00 - $28.00 per hour; generous paid time off; 11 paid holidays per year; flexible scheduling; medical, dental, vision, life insurance, AD&D, short-term and long-term disability, and retirement plan; immediate eligibility for matching in the retirement savings plan, immediate vesting of employer contribution.

Responsibilities

  • Provide comprehensive case management to pregnant women and new mothers including assessments, care plan development, and weekly check-ins.
  • Facilitate access to community resources and support services, including healthcare, mental health services, and social assistance programs.
  • Host regular drop-in groups to foster social connections, provide support, and facilitate enrollment in structured classes and case management services.
  • Organize weekly stress reduction classes (eg, yoga, meditation, somatic exercises, etc.) in collaboration with community professionals.
  • Recruit and engage community experts and elders to serve as mentors to expectant mothers and new moms.
  • Plan community gatherings, special events, and celebrations.
  • Maintain accurate and up-to-date documentation of program engagement, activities, and outcomes in SalesForce.

You are a good fit for this role if you are...

  • You hold a high school diploma or GED and have four years of experience in human services or community outreach, or you hold an associate degree with two years of experience in human services or community outreach.
  • You have or are willing to enroll in an accredited CHW and/or doula certification program and complete it within one year from the date of hire.
  • You have a demonstrated ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, and economic circumstances.
  • You have the ability to build and develop constructive working relationships with a variety of individuals.
  • You have the ability to execute outreach plans, collect data, and prepare reports.
  • You have excellent verbal, written, analytical, and group communication skills, preferably with experience in a multi-cultural environment.
  • You are proficient in MS Outlook, MS Word, MS Excel, and databases.
  • You have knowledge of professional standards in delivering exceptional customer service when addressing inquiries or requests from clients, visitors, volunteers, and the community.
  • You have a demonstrated ability to advocate and utilize tact, diplomacy, professionalism, and empathy in interactions.
  • You have the ability to follow data privacy and confidentiality standards and protocols.
  • You are skilled in taking initiative to ensure all standards/protocols are met and/or exceeded.
  • You have a valid driver's license, access to reliable transportation, and the ability to meet Sabathani's driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
