Job Summary

This position provides technical assistance, coaching, and training to the CareOregon community-based organization (CBO) network in areas related to CBO service delivery of social health services, as well as related payment, system integration, and liaison activities for organizations in the social health network, both contracted and potential partners. This role requires a high degree of collaboration, coordination, and influence with multiple internal and external stakeholders, and all work will be conducted through the lens of improving health equity for all vulnerable populations.

Essential Responsibilities

Partner Support & Technical Assistance

  • Develop and maintain effective working relationships across the social health network.
  • Obtain and review partner indications of contract interest and service contract bids; support regional community health leaders in identifying potential partners to enhance the network, and in determining partner readiness, fit, and scope of services.
  • Conduct new social health payment partner orientations; train partner staff on CareOregon administrative processes.
  • Provide technical assistance to partners on the development of reliable workflows and processes related to service delivery operations, service documentation, payment activities, reporting and compliance requirements.
  • Participate in the development of community-based delivery of health services for the social health network.
  • Conduct site visits as needed to support and maintain effectiveness, deliver technical assistance, and participate in audit and improvement activities.
  • Research partner questions and connect them to appropriate resources; coordinate resolution of root cause of partner issues and concerns.
  • Assist with the contract development process and general administration related to those contracts.
  • Review and provide guidance related to contractual issues, complaints, and concerns in assigned areas.

Internal Liaison

  • Participate on interdepartmental initiatives and teams.
  • Attend CareOregon events, serving as an integral team member.
  • Work closely with other internal departments to help address processes that affect partners and support the resolution of issues.
  • Provide support on planning and facilitating collaborative meetings.

Project Management

  • Lead a variety of small improvement projects relating to process development and optimization with network partners; projects may span across multiple geographies.
  • Using project management knowledge, identify appropriate teams, including internal staff, external partners, and community groups required to meet project goals.
  • Compile assessment information, document needs, and create work or project plans.
  • Apply appropriate improvement tools and technology essential to meeting project goals and communicate with other departments as directed.
  • Establish and ensure effective channels of communication for reporting on project status, coordinating changes, and making decisions.
  • Collaborate with others to ensure the coordination of development and planning efforts between multiple external community organizations and internal departments.

Administration

  • Contribute to relevant policies and procedures.
  • Support administrative tasks related to partner information sharing and partner reimbursement as needed.
  • Provide information on CareOregon program development and initiatives.

Experience and/or Education

Required

  • Minimum 3 years' experience relating to community-based organizations or the delivery of social health services. Experience should include work in one or more of the following areas operating with independence: Public health, population health/design, health literacy, community-based organization operations, or project/program administration

Preferred

  • Experience related to social health payment innovation, in particular those developed for use in the Oregon Health Plan for Coordinated Care Organizations

Knowledge, Skills and Abilities Required

  • Knowledge of managed care and the Oregon Health Plan
  • Understanding of community-based organization operations and services

  • Ability to effectively identify deficiencies or delivery problems, and in partnership with network partners develop action plans and implement solutions
