Job Summary

Under the direction for the supervising social worker, the clinical social worker will engage with individuals living with HIV (or affected by HIV) who are unsheltered or experiencing unstable housing conditions. This dynamic role involves providing comprehensive social work services within a drop-in primary care clinic known as Positive-health Program for Unstably housing Populations (POP-UP and Health Access Point (HAP). The clinical social worker will establish meaningful connections with homeless individuals and theirfamilies with the purpose of securing stable housing byoffering social service referrals and facilitating access to clinical services and relevant community resources. You will play a crucial role in assisting individuals and families throughout the housing application process, ensuring the acquisition of necessary documentation, and establishing benefits for successful housing placement. The clinical social worker will provide compassionate and supportive interventions to address psychosocial issues, enhance overall functioning, and help individuals develop insights that reduce stressors and distress. Your support will extend beyond securing housing, as you will also assist those who are already housed in adjusting to independent living and accessing community resources essential for maintaining housing stability. The clinical social worker will provide basic education about antiretroviral treatments, supporting individuals in initiating and maintaining consistent adherence. By reducing transmission risks and minimizing medical complications associated with HIV, you will make a significant impact on improving overall health outcomes.
Collaboration is key in this role, as you will work closely with an interdisciplinary healthcare team and contracted agencies to support patient stabilization and ensure access to vital supportive services. Your active involvement in weekly case conferences, staff meetings, in-service trainings, and other mandatory clinical meetings will foster a cohesive and informed approach to care delivery.

Department Description

The Division of HIV, Infectious Diseases and Global Medicine at the University of California, San Francisco (UCSF) is an internationally recognized global leader in clinical care, research, and education. With its home base located at Zuckerberg San Francisco General Hospital (ZSFG), the Division has been ranked as the top facility in the country for AIDS care for over a decade. The Division's work is featured frequently in the media, and it is often visited by international, national, state and local dignitaries and delegations. The Division consists of approximately 36 faculty members and 140 academic and staff employees. It is the largest Division of the Department of Medicine (DOM) at ZSFG, with a total expenditure budget of approximately $45 million dollars.

Required Qualifications

* A Master's degree from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
* Possess knowledge and is sensitive to the needs of the homeless population
* Familiar with San Francisco housing, community-based resources, and services
* Two (2) to four (4) years of social work experience providing services to HIV positive populations that are diverse in gender, sexual orientation, language, country of origin, race, and ethnicity.
* Ability to work in a high stress, high volume area and work well under pressure.
* Ability to function independently as a collaborative, supportive team member.
* Must demonstrate cultural humility and a strengths-based, recovery philosophy.
* Demonstrated excellent oral/telephone communication skills and written documentation.
* Must be computer-literate, working knowledge of electronic medical records systems, and Microsoft Suite.
* Ability to handle demanding workload involving multiple tasks effectively and efficiently.
* Excellent organizational and strong interpersonal skills
* Demonstrated excellent attendance and reliability

Preferred Qualifications

* HIV Test Counselor
* Licensed Clinical Social Worker; or Licensed Marriage Family Therapist
* Fluency in Spanish - ability to communicate effectively with monolingual patients

License/Certification

* Possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners
